Budd Chiari is not a lack of blood entering the liver. It is a failure of blood leaving the liver through the hepatic veins or IVC. That is why congestion, hepatomegaly and ascites dominate.

The caudate clue matters. The caudate lobe can enlarge because it has more direct venous drainage into the IVC, while the rest of the liver is congested by hepatic vein obstruction.

Moderate thrombotic obstruction. Confirm flow, anticoagulate and search for a prothrombotic driver.
